What is an NDC code?
A National Drug Code is the FDA’s product identifier for human drugs in the United States. Every drug marketed in the US is listed with the FDA under an NDC. The number identifies a product and its package; it is not an approval, and a listing does not mean a drug is covered or clinically appropriate. More on what an NDC is.
How NDC numbers are structured
An NDC has three segments, separated by hyphens in the FDA’s 10-digit form:
- Labeler code (4 or 5 digits), assigned by the FDA to the company on the label. See the labeler directory.
- Product code (3 or 4 digits), assigned by the labeler to one strength and dosage form.
- Package code (1 or 2 digits), assigned by the labeler to one package size.
The labeler and product segments together are the product NDC; all three are the package NDC. NDC format in detail.
10-digit vs 11-digit NDC
The FDA publishes NDCs as 10 digits in a 4-4-2, 5-3-2 or 5-4-1 shape. HIPAA claims use an 11-digit 5-4-2 form, made by adding a leading zero to the short segment. Each product page on Med-Code shows both. How to convert.
How to find an NDC number
The NDC is printed on the drug’s package and label. To look one up, search by drug name, ingredient or labeler above, or browse the drug and ingredient directories. How to find and verify an NDC.
NDC vs HCPCS and J-codes
An NDC identifies a product and package. A HCPCS J-code describes a drug and a dose unit for billing, whoever made it. One J-code usually covers many NDCs, and drugs dispensed by a pharmacy are billed by NDC with no J-code. NDC vs J-code · J-code list.
NDC codes in medical billing
On a professional or institutional claim, a drug line carries the HCPCS code and units, plus the 11-digit NDC with a unit qualifier and quantity. NDC billing and units of measure.
